PURPOSE :
Health Legacy of Cleveland, in partnership with the greater Cleveland community, is dedicated to supporting professional education and to eliminating health care disparities. By awarding scholarships to Under Represented Minorities in Medicine (URMM), African-American physicians and dentists who practice in Northeast Ohio.
Health Legacy of Cleveland will award scholarships on a competitive basis to medical and dental school students who have submitted the completed scholarship application by May 1, 2012.

ELIGIBILITY :
• African-American medical and dental students attending HBCU's: Howard, Meharry, Morehouse; Ohio Schools: Case Western Reserve University or North East Ohio Universities College of Medicine (NEOMED)
• Show proof of acceptance or enrollment in a partner school for the 2012-2013 academic year
• Demonstrate a commitment to return to practice in the Northeast Ohio community
• Submit the completed essay and application by May 1, 2012

HOW TO APPLY :
• Complete the Scholarship Application Form and required Essay (Renewing scholars do not need to complete the essay)
• Two letters of recommendation from individuals other than family members
• A sealed, certified copy of undergraduate and/or graduate transcripts
• A letter from the financial aid office verifying financial need
• An essay of 500 words or less answering the questions on the form (Renewing scholars do not need to complete the essay)
• Submit completed application packet by May 1, 2012. Late arrivals will not be considered.
